Medical Phy “REAVIZ”
1. The original document of the state sample about education or its photocopy.
2. Russian citizen passport or a photocopy.
3. Conclusion of the medical organization on the results of a preliminary medical examination entering the volume stipulated claim 17 Appendix №2 to the orders of the Russian Federation Ministry of Health and Social Development on 12.04.2011, № 302n;
4. 4 photographs (3×4 cm).

For foreign citizens:
1. The foreign citizen passport
2. A photocopy of the passport translation into Russian
3. Original (or a photocopy) of the document state education (duly certified translation into Russian of the document on the level of education), a copy of the certificate of recognition of the document
4. Conclusion of the medical organization on the results of a preliminary medical examination entering the volume stipulated claim 17 Appendix №2 to the orders of the Russian Federation Ministry of Health and Social Development on 12.04.2011, № 302n;
5. 4 photographs (3×4 cm).
